Willow and Bloom Wellness ("we," "us," or "our") is the private practice of Emily Niess, LIMHP. This Privacy Policy describes how we handle information on this website (https://www.willowandbloomwellness.net), including information you submit through the contact and appointment forms.

Contact and appointment forms

The contact form and the appointment request form are provided by SimplePractice and embedded on this site. We do not operate those forms ourselves.

Information you submit through them—such as your name, email address, phone number, and the content of your message or appointment request—is sent to SimplePractice so we can receive it and respond. That information is subject to the SimplePractice Client Portal Privacy Policy.

What we collect

When you use the contact or appointment form, the information collected is what you choose to enter. That may include:

• Your name
• Your email address
• Your phone number
• The content of your message or appointment request

This website does not set its own cookies, and it does not use analytics or advertising tools. Cookies may still be set by the embedded SimplePractice forms, as described below.

Cookies

Because the contact and appointment forms are embedded from SimplePractice, loading or using them may set cookies or similar technologies in your browser. Those cookies come from SimplePractice, not from this website. They are used to operate the forms. This site does not use them for advertising or analytics.

• client-portal-session keeps a session for the embedded client portal. It stores a session identifier and whether you are signed in, so the form can keep your visit together while you use it.
• client-portal-session-expiration_time stores how long that session lasts. It is set to 86400 seconds (24 hours).
• tracking_enabled records whether SimplePractice tracking is turned on. It is set to false, so tracking is off.

You can block or delete cookies in your browser settings. If cookies are blocked, the embedded forms may not work. SimplePractice describes its own use of cookies in the privacy policy linked above.

How we use your information

Information submitted through the contact and appointment forms is used to respond to your inquiry or appointment request. Submissions are received through SimplePractice, as described above. We do not sell your information, and we do not share it with anyone else except as required by law.

This form is not a clinical channel

This contact form is not a secure, HIPAA-compliant clinical communication channel and should not be used to share protected health information (PHI), crisis situations, or anything you consider sensitive medical information.

If you are experiencing a mental health emergency, please call 988 (Suicide and Crisis Lifeline) or go to your nearest emergency room. Do not use this form for urgent matters.
